Tell your family, friends and coworkers about Hoye Dental, and if they become new patients, you and your referred patient(s) will each be eligible to receive free upper and lower bleaching trays and 1 tube of bleach (a $300 value). For each set of bleaching trays (upper and lower) and 1 tube of bleach, the patient must be new to Hoye Dental, or the referrer must be a current patient at Hoye Dental. The new patient must complete the initial appointment. The referrer and the new patient will each be eligible to receive bleaching trays and 1 tube of bleach upon completion of the appointment. For each patient referral raffle entry, the referrer must be a current patient at Hoye Dental. The referee must give the name of the referrer when scheduling the initial appointment and complete the appointment. The referrer and the referee will each be entered once into the raffle for a total of two entries upon completion of the appointment.One gift certificate in the amount of $100.00 to Village Fish Restaurant will be awarded to one raffle winner. A representative from Hoye Dental will notify the winner during the first week of each month. The referrer can be entered into the raffle for an unlimited number of entries, for however many individuals they refer to the practice (2 individual referrals equals 2 entries for the referrer, and 1 entry per each referee for a total of 4 entries). All raffle entries must be received between the first and the last of the month to ensure eligibility for the raffle drawing at the beginning of the following month. Referrals given prior to the present month are not eligibile (example: pt. referrals from March are not entered into the June drawing.)
